授業評価アンケートにご協力ください
2007年度に、全学共通科目の授業評価アンケートを、WEBアンケートシステムにより実施することとなりました。学生の皆さんの積極的な姿勢が授業改善の第一歩となりますので、ぜひご協力をお願いします。2007年度春学期の実施方法等は、次のとおりです。

全学共通科目（春学期開講科目） 203科目

対象科目の授業時に、アンケート回答に必要なコードを記載した「授業評価アンケート回答チケット」を、1人1枚配布します。
| STEP1 |
アンケート実施期間（7月9日〜31日）に、パソコンから、「授業評価アンケート回答チケット」に記載されているWEBサイトのURLにアクセスしてください。24時間、アクセス可能です。 |
| STEP2 |
上智大学授業評価アンケートWEBのトップページから、「授業評価アンケート回答チケット」に記載されている16桁のアンケート回答コードを入力、送信し、アンケート回答画面に進んでください。アンケート回答コードが間違っている場合は、エラーメッセージが表示されます。コードを確認して、再入力してください。 |
| STEP3 |
アンケートの質問項目に従って入力し、回答後、送信をクリックしてください。これで終了です。一度アンケートを送信すると、アンケート回答コードを再度使用することはできません。なお未回答項目があっても、送信可能です。 |

* アクセスの集中を防ぐため、回答チケットを受領した後できるだけ早く回答いただきますよう、ご協力をお願いします。

2007年度秋学期に同様のアンケートを実施します。秋学期の実施方法等については、再度、11月頃にお知らせします。 |

アンケートは無記名で、回答者が特定されることはありません。また、アンケートの回答内容が成績評価に影響することは一切ありません。 |

Student Course
Evaluation Questionnaire
We need your cooperation!
In the Academic Year 2007, a Course Evaluation Questionnaire
for General Studies Courses (zengaku kyotsu kamoku)
will be carried out.
We ask all students to participate. Your cooperation
will lead to improvement of the classes.
Please read the following for the spring semester evaluation.
【Courses to be evaluated】
General Studies Courses: 203 courses (offered in Spring
semester)
【Procedure】
How to access and reply to the questionnaire:
※ You can also access from off-campus PCs.

At class, each student attending the class to be evaluated
will be given a Course Evaluation Questionnaire Reply
Ticket with a code required to access and reply.
| STEP 1: |
Access the web site URL printed
on the Course Evaluation Questionnaire Reply Ticket
during the reply period (July 9- July 31, 2007).
Access is available 24 hours. |
| STEP 2: |
From the top page of Sophia
University "Course Evaluation Questionnaire"
site, enter and submit the 16-digit questionnaire
reply code printed on the "Course Evaluation
Questionnaire Reply Ticket". Then proceed to
the reply page. If the code is incorrect, an error
message will appear. Check the code and try again. |
| STEP 3: |
Answer the questionnaire and
click on "送信send." This completes the
questionnaire survey procedure. Once the questionnaire
is sent, the code cannot be used again. The questionnaire
reply is accepted with some unanswered questions. |
【Evaluation survey period】

"Course Evaluation Questionnaire
Reply Ticket" will be distributed:
Monday, July 9- Friday,
July 13, 2007.
* The Ticket will be given at class by the course
instructor of the course to be evaluated.
* Depending on class situations, the Ticket may
be distributed one week later or earlier. |

Questionnaire should be replied:
Monday, July 9- Tuesday,
July 31, 2007
* To avoid access congestion, please reply as soon
as possible. Thank you for your cooperation. |
【Others】
| (１) |
The same questionnaire survey
will be conducted for the 2007 Autumn semester.
The details will be announced around November. |

The questionnaire
is answered anonymously. Your reply will NOT affect
your grades. |
